The Bank of Industry (BOI) has asked some applicants whose MyBOI customer onboarding applications were returned to provide additional information and upload required forms before processing can continue.

In an email sent to an applicant, BOI directed customers to log in to the portal and complete tasks under the Regulatory Information and Stakeholders tabs. The notice says applicants must download, complete and upload the Account Opening Mandate Card and Signatory Card in their respective fields, select the appropriate stakeholder type where required, and resubmit the application.
A returned application does not automatically mean a loan has been rejected. According to the MyBOI portal’s frequently asked questions, the status means corrections, clarification or additional information are required. Applicants can edit and resubmit their applications.
The portal’s Mandate Card section instructs applicants to upload a separate completed copy for each signatory and use the “Add Document” button where necessary. The Account Signatories section similarly requires a separate Signatory Form for each signatory.
Applicants should ensure that names and business details match their registration documents and that all required fields are completed accurately. The mandate card includes account and organisation details, signatory information, identification details, photographs, signatures and the applicable signing arrangement.
How to upload
- Log in to the official MyBOI portal and open your returned application.
- Under Regulatory Information, download the Mandate Card and Signatory Form.
- Print and complete both forms according to the instructions.
- Upload each completed form in its correct section.
- Add separate copies for every signatory, where required, using the “Add Document” button.
- Under Stakeholders, select the appropriate stakeholder type.
- Review your dashboard for other outstanding items, save your changes and resubmit the application.
- Use only official BOI channels and never share passwords or one-time passwords (OTPs).
